LPN or RN NURSE CASE MANAGER

8:00 am – 5:00 pm

Monday – Friday

This position also requires the ability to:

  • Oversee the day-to-day delivery of health services provided to all short-term rehab residents.
  • Possess excellent assessment and clinical skills.
  • Ability to communicate medical information in terms residents and families understand.

Wound Nurse-Part-time days

Full-time, Part-time and PRN QMA, 1st & 2nd Shift

  • Must have Indiana CNA certification
    • Must have Indiana QMA certification
  • Previous Healthcare, Geriatrics, Caregiving, or experience working with the elderly population preferred.
  • The Qualified Medication Aide sets up, administers, and records certain prescribed medications for residents in the facility under the supervision of a licensed nurse and as allowed by State regulatory agencies.

Dietary — Part Time — Day- Afternoon Shift

Dietary Supervisor,

  • 6:00 am – 2:30 pm
  • CDM certification preferred

Primary Function – Assists the Director of Nutritional Services in the overall management of the Nutritional Services Department in accordance with current applicable federal, state and local standards guidelines and regulations as directed by the Director of Nutritional Services.

Job Requirements:

  • Duties include setting and clearing tables, delivering and serving food to residents from the kitchen in a timely efficient, friendly fashion and performing various cleaning tasks and side work. Must be friendly and able to coordinate multiple tasks.

Hostess / Host, Part-time, 3:45 pm – 7:15 pm

Cook-Part time, 5:30 am – 12:30 pm

Job Specific

  • Maintains department standards for food temperatures.
  • Tastes all food items produced (including puree & mechanical) and seasons according to the recipe.
  • Prepares all food items according to a standardized recipe for an item in quantity indicated on the production sheet.
  • Informs supervisor and documents any and all changes made in a recipe.
  • Analyzes quantities noted on the production sheet and communicates with a supervisor about any inconsistency based on previous menu cycles.
  • Serves hot food from the steam table for residents receiving the meals via tray service.
  • Records leftovers and run-outs on production sheets.
  • Prepares additional food – batch cooked as possible – throughout the meal service period.
  • Suggests, prepares, and records substitutions when shortages occur.
  • Prepares food in a timely manner to assure service of meals at scheduled times.
  • Follows and documents HACCP cool-down procedure.
  • Demonstrates knowledge of acceptable raw and cooked product quality.
  • Completes daily, weekly, and monthly cleaning tasks as indicated on the cleaning schedule.
  • Maintains “clean as you go” standard to keep the work area clean and sanitized.
  • Reports hazardous conditions/safety concerns to the supervisor immediately.
  • Notifies supervisor immediately if the product needed is not available or equipment is not functioning properly.
  • Attends at least 50% of department meetings. Reads and signs minutes of those missed.
  • Prepares items needed for special events.
  • Covers, labels and discard dates all items produced or portioned.
  • Follows hand washing procedure while preparing and serving food, after answering the telephone, etc.
  • Consistently maintains a clean, well-groomed appearance and follows department policy for uniform.

Hospitality Associate 2 Full Time, 9:00 am – 5:00 pm

Part time Wait Staff: 3:45 pm – 7:15 pm– Strong customer focus is necessary.  The primary function is serving resident meals in the Main Dining Room including but not limited to, communicating the menu for the meal, taking orders for each resident, serving food to residents.

Part time Dietary Aide: 7:30 am – 3:30 pm-Performs a variety of kitchen duties such as dish room operation, works on try line meal assembly, prepares special items for meal service, and general cleaning assignments.

Part time Dietary Aide: 4:00 pm – 7:30 pm -Performs a variety of kitchen duties such as dish room operation, works on try line meal assembly, prepares special items for meal service, and general cleaning assignments.

Part time Cook: 12:30 pm – 7:00 pm
