Housekeeper,-

7:00 am – 3:30 pm

Every other weekend

Supervisor experience preferred

Cleans and services the facility under the direction of the Housekeeping Supervisor.  Maintains facility in a clean, sanitary, orderly, and attractive condition to provide a suitable environment for the care of the residents

Nursing Supervisor- Part-time nights

Must possess the ability to share knowledge of the needs of the elderly, disabled, and confused residents.  Display the ability to identify and promote restorative measures.  Demonstrate the ability to acquire, maintain, and convey knowledge of pertinent state and federal regulations applicable to the long-term care setting.

Areas of Responsibility:

  • Function as facility supervisor.
  • Monitor the quality of resident care given.
  • Function as a team member of nursing administration.
  • Maintain professional competence.
  • Carry out disciplinary actions/counseling as directed by the assistant directors of nursing or the director of the nursing department according to the Healthwin Specialized Care corrective actions policy and procedure.
  • Adhere to safety and infection control policies and procedures.
  • Practice and provide guidance for performance improvement.
  • Perform all additional duties as assigned.

Shower Aide

Monday thru Friday

5:30 am – 1:00 pm

  • Must have Indiana CNA Certification
  • Provide showers/tub baths according to resident preference
  • May work alone or with assistance of another Nurse or CNA
  • Answer resident call lights to meet resident needs
  • Transport residents using wheelchair, lifts or other assistive devices
  • Reports changes in resident bathing schedule
  • Sanitizes shower chairs and tubs between showers
  • Assist unit nursing staff as needed

Health Information Management (HIM) Clerk

  • Conducts quantitative audits on special areas of documentation in open resident records as directed by the HIM Manager.
  • Assists in maintaining and requesting transfer information necessary for continuity of care purposes from the community health care institutions.
  • Assist in thinning/purging health records on the nursing units.
  • Accurately enters identification information into the EMR at the time of admission and on an on-going basis.
  • Maintains an organized workflow of documents on the nursing units and in the HIM office.
  • Performs routine maintenance of the current active nursing unit chart as per the established facility policy.
  • Maintains organized overflow/depleted files beyond the required retention period.
  • Assist in maintaining the flow of reports to the resident health record in response to physician orders.
  • Promptly scans/files all diagnostic, therapeutic, physician orders and continuing care sheets in resident record and EMR.
  • Delivers the health record of discharged resident to the Health Information Department within the established time frame.
  • Process the discharge record in the established uniformed arrangement, gathers all component at the time of discharge.
  • Assist with the assembly component of discharge and conducts timely audits for admissions and discharges.
  • Maintains adequate stock/supply levels needed for resident care needs.
  • Maintains confidentiality of all resident related information, whether verbal, written, or based upon observation, regardless of its origin.

Dietary — Part Time — Day- Afternoon Shift

Job Requirements:

  • Duties include setting and clearing tables, delivering and serving food to residents from the kitchen in a timely efficient, friendly fashion and performing various cleaning tasks and side work. Must be friendly and able to coordinate multiple tasks.

Part time Wait Staff: 3:45 pm – 7:15 pm– Strong customer focus is necessary.  The primary function is serving resident meals in the Main Dining Room including but not limited to, communicating the menu for the meal, taking orders for each resident, serving food to residents.

Part time Dietary Aide: 7:30 am – 3:30 pm-Performs a variety of kitchen duties such as dish room operation, works on try line meal assembly, prepares special items for meal service, and general cleaning assignments.

Part time Dietary Aide: 4:00 pm – 7:30 pm -Performs a variety of kitchen duties such as dish room operation, works on try line meal assembly, prepares special items for meal service, and general cleaning assignments.

Part time Cook: 12:30 pm – 7:00 pm
